All parents and volunteers over the age of 18 must complete the required free clearances before assisting with field trips, volunteering for school activities, or participating in school events. (If you already have these for work or another purpose, please turn in copies to the office.)

First
Clearance

Child Abuse
Fill out the form online, print it, and mail it in.

OR  Create (or access an existing) account, complete and submit it online
Access
Submit a copy to the office upon receipt.

Second
Clearance

PA State Police Background Check
Complete and submit online
Download
  1. New Volunteer Record Check
  2. Check the box and Accept
  3. Fill out the form and submit
  4. Send Jessica Armstrong jarmstrong@gohca.org a copy of the email they send you that includes your Control Number

Third
Clearance

FBI Fingerprinting
IF you have lived in the state of PA for at least 10 years : Volunteer Form: sign and return this form

  1. Set up an appointment for fingerprinting. FBI Fingerprinting (pa.gov)
  2. When scheduling an appointment, you’ll be asked for a service code — please contact the office to obtain it.
